Is my tree dying?

This is year three for this tree.. looked super healthy in the spring, but leaves seem super dry now and falling off early. I noticed that some of the leaves seem to perferated, some type of pest? It also seems to be wanting to grow straight up, should I be trimming those top shoots? Its a red maple and im in Southwestern Ontario.
