Pothos

Help! My pothos are dying

I repotted my pothos using a new soil mix about 3 weeks ago. I thoroughly watered it about 15 days ago but today I checked and the pot was heavy, so I decided to take it out and see what was going on. The roots are rotting and the leaves are turning yellow. When I touched the soil it was still moist from 15 days ago.

What should I do? What type of soil should I have used?

Just for reference the mix I did was equal parts:
Perlite
Sphagmum peat moss
Earthworm castings
Orchid bark

Except for those two leaves, the other plants seem fine to me. The substrate looks ok, how about the pot drainage? I bet that’s the problem. It’s normal that some leaves die after repotting due to stress.

Edit: the peat moss tends to retain too much water, maybe add more soil or perlite?
