The flowers have one by one fallen off, a big leaf fell off and another one is starting to go yellow.
It is potted in orchid mix, I don't water it too often, soil is damp but not wet. Gets plenty of indirect sunshine during the day.
Please help, first time orchid growing failing here.

The potting mix looks a little dry but it is very normal for the bigger leaves to yellow and fall off when it’s inside a smaller pot
Hard to say. In your pictures, the roots look dead – gently pull the orchid out and have a look. I suspect there’s probably a moisture or oxygen issue, but again, there’s not enough info here to diagnose.
