Poor soil/saturated too long?

I haven’t encountered this in my plants before. This painted lady philo was a clearance rescue that I’ve had for about 2 months. The soil was dried out when I bought it and it was overall in rough shape, so I watered it but didn’t repot it right away to avoid further stress. It’s still in the original soil. The leaf is mushy in the immediate area around the hole, but the rest of the leaf and the plants in the pot look healthy. The hole itself was created when I touched the spot and it disintegrated. I honestly can’t remember when I watered this last (probably a few weeks), but I had waited for the soil to almost fully dry out again. Still pretty damp as of today. The grow light has been on it for 9 hours every day, but as of yesterday I bumped it to 12. I suspect it just needs to be repotted into chunkier soil, which I will do when I have time tonight. Just wanted to see if this could be anything else as this is also my first time using a grow light for a plant, though this doesn’t look like light-related damage to me.

You may have root rot going on. I would pull the plant out of the pot and check. Treat for root rot if you notice any black, mushy or smelly roots. If the roots look good, repot in a chunky aroid mix. In my experience, philodendrons do not like to stay wet/moist for very long.
