The things IS opuntias are thin and tall so it often just falls to its side.

The first grafts I did were onto opuntia. I found it was easier to hand and graft to when I sandwiched the cactus between 2 pieces of cardboard.

I did x type of grafting on my christmas cactus
(Also cut out a little wedge from one and the same shape rom other. And placed that wedge into the cutted part. Was pain in one place to do, but it was successful graft)
Billeats
I recommend cutting it like this, sticking the pieces together and securing with clips or rubber bands. This way you don’t need downward pressure, just pressure from the sides.

I’ve stumbled onto a couple of videos, maybe they will give you some ideas/inspiration. I imagine you could combine these 2 techniques in a couple of ways to avoid punching holes in the cacti.

I haven’t done opuntia on opuntia, but I think something like this would work. You can move the lower rubber band up and down to adjust the pressure
