Not sure what it is. But here’s what I would do. Trim off the bloom, put it in a plastic bag and shake out and save the seeds (more plants!). Make sure that it has water but doesn’t stay too damp. I would not try to repot it until it looks much happier and perks up. Also trim off the yellow/dead leaves. Gently wipe leaves to remove any potential pests, etc.

I think it’s an azioaceae, “Fig Marigold” or “Ice Plant” and my guess is Glottiphyllum depressum
Needs lots of light, but not a lot of water (basically a succulent). Make sure soil is well-draining. Looks in pretty good shape, it can come back easy.
